Service Agreement.

This agreement covers basic information that we feel is necessary for you, the client, to be aware of when leaving your pet in our care. Please read each section thoroughly and digitally sign by clicking the button at the bottom. You will only have to sign this form once, unless edits are made in the future. Feel free to reach out if you have any questions.

Cancellation Policy - If you need to cancel or reschedule your pet's appointment, you must contact us at least 24 hours in advance. If we are not given at least 24 hours' notice, or if you do not show up to your appointment, you will be charged a $20 no-show/late cancellation fee.

Late Drop Off - Pets brought in more than 15 minutes after their appointment time are considered late and may have a $15 late fee added to their service. We will accept late drop-offs on a case-by-case basis up to 30 minutes past the scheduled appointment time. Clients who arrive more than 30 minutes late will be considered a no-show and are subject to our cancellation policy. We typically do not accept pets more than 30 minutes late.

Late Pickup - You will be notified when your pet is ready for pickup, either through text message or phone call. If your pet is not picked up within 2 hours of receiving this notification, they are subject to either a late pickup fee or a day board charge applied to the service. If you do not pick up your pet before closing time (5pm EST) you will be charged an after hours pick up fee.

Vaccination Requirements - It is Ga State Law that all dogs and cats must be vaccinated for rabies. By signing this agreement you are ensuring that your pet is up to date on their rabies vaccine and will be able to provide veterinary documents showing this upon request. We will also be allowed to call your vet to verify that your pet's vaccination information is up to date.

Pet Health - It is important that any health issues your pet may have be disclosed prior to the beginning of any service. Even if you think it may not affect the groom, it is still important for us to know about. Dog Stars is not responsible for any incidents caused by a pre-existing health condition.

Pet Behavior - You must let us know ahead of time if your pet has any behavioral issues, including but not limited to anxiety, aggression toward animals or people, nervous peeing, et al. We may stop a service at any time if we believe continuing the service will cause harm or trauma to your pet or the groomer.

Pet Injury - Working with live animals means that interactions are not always predictable. A pet may move suddenly or try to bite, or the groomer may accidentally nick an ear. Accidents happen, and while we will never harm your pet on purpose (just like we know your pet would never purposefully harm us), it does happen in this industry.

If your pet is injured in our care, you agree to allow Dog Stars Staff to become your pet's agent in medical decision making, including taking your pet to a vet for treatment. If the ailment is declared by the veterinarian to be caused by a pre-existing condition, Dog Stars LLC will not be responsible for the cost of veterinary care.

Refunds - Please reach out to us within seven (7) business days of the appointment in question if you are unhappy with your pet's service. If we are unable to remedy the situation, then we may issue either a partial or full refund, or we may offer a credit on your next service. We do not issue refunds if complaints are made after seven (7) business days.

Matting - If a pet in our care is severely matted, it is in the pet's best interest to shave the matted fur off. Sometimes this means shaving the entire pet from head to toe. Matting can cause a pet's circulation to become restricted, which can result in bruising, skin irritation, skin infections, or even hematomas in the ears (collection of blood in the flaps of the ears from the sudden relief of constricted blood flow due to shaving off mats). You will be given an excessive matting information sheet to take home if your pet is shaved due to mats. Hair matting is considered a pre-existing condition.
